An employee was directing an operator in setting a catch basin at an excavation site when the catch basin hit an excavation wall and one of the chains came loose, allowing the catch basin to fall approximately 3 feet onto the employee's leg. An employee was riding a horse when it saw or smelled a mountain lion and spooked, bucking twice. On the first buck, the employee's face smashed into the crown of the horse's head. On the second buck, the employee was thrown to the ground. The employee suffered blunt force trauma to the facial area, including a broken jaw, sinus damage, and three broken bones near the right eye. The employee also sustained a broken right wrist upon hitting the ground.
